Step 5 Data Infrastructure and Data Warehousing for PHM
The Data (Management) Infrastructure capability encompasses the specification, sourcing, exchange, storage and processing of useful, high-quality data that is used to generate population health intelligence
There are three stages of maturity (in increasing maturity):
- 1. Data only available about individual care settings aggregated across the system, no ability to link person level data sets.
- 2. High quality data available and capable of being linked in ways that support effective analysis, with appropriate legal basis, across several settings of care (primary, secondary, social care, ambulance, community). Some real time data.
- 3. High-value and quality data comprehensively available, linked to the priorities of the system, integrated across all settings of care including data sets that inform understanding of wider determinant of health. Data are capable of being linked in ways that support effective analysis. Real time data and patient tracking available.
All Clusters should do a PHM data needs and maturity assessment and include the requirements for this within their Cluster HIS RFP
As with digital maturity, whilst the long-term aim is to reach full maturity, there are short- term solutions that are able to help systems start with basic PHM

Key questions to ask on the Data Infrastructure?
- 1. What are the assumptions that I need to make about my population priorities based on current available data?
- 2. What data sources we can trust and what correction factors can we use?
- 3. How to ensure the data infrastructure will be set up correctly going forward to support PHM?
- 4. What are the key minimum data sets that will drive my PHM interventions and do I have them in my IT road map systems design and procurement?
- 5. What IT ad data repository I need now and in future to focus on the high impact areas based on a PHM approach?
