How to use the manual

  1. It is expected that clusters will use this as a planning and guiding tool in close collaboration with their Hubs, MOC teams and other parts of the transformation program. PHM involves the effective use of all resources at all levels of care. This is not to be done in isolation as a separate PHM project

  3. The Manual has been organised in different stages and steps of the PHM methodology and each of these are hyperlinked for quick access by clicking on them

  5. Where required external references and links have been provided for further information, international benchmarks, online tools etc
  6.  
  7. The manual provides a tested and proven approach and each cluster can adopt and modify the Stages and Steps as required to fit local circumstances

  9. There are different calculators and tools embedded within each stage for the clusters to down load and use. These are based on assumptions and projections as discussed and agreed in the PHM working group and will be revised and updated as we get more data and inputs from different sources
