Step 2 Population Definitions

We must define what population is being addressed before we can start managing its health and wellbeing. A system population should be:
  • A meaningful geographic footprint that respects patient flows
  • Contiguous with local authority boundaries, or – where not practicable – clear arrangements for working across local authority boundaries
  • Covers one or more existing zones/clusters, with a population of ~1m or more
The aim is to eventually have a shared understanding of the population and preventative measures in place at the system, place and neighbourhood levels, with priority target groups identified, based on evidence.

Key questions to answer

  • 1. How will you define and validate your total population size?
  • 2. How will you identify correction factors for population size including cross border transfer, national services etc
  • 3. What is the role of ELM/ Absher in defining the population size and profile?

Once we have a clarity on the population we serve we need to look at the “Understanding Population Need” which will be described in detail in the next section of the manual
