Step 6 PHM team structure - Capacity and Capability
Delivering an end to end PHM program needs multiple capabilities and the cluster needs to ensure they develop all such skill sets. The below is the PHE framework from NHS which can be used as a model to ensure different types of skill sets are made available form existing teams:
In addition to the above PHM data analysis and interpretation the cluster needs to work with following teams to develop the interventions to be implemented:
- 1. MOC COE for pathway development and standardisation
- 2. Finance transformation team to costs and drive value
- 3. PHAP teams for various payor and policy developments
- 4. MOH IT and eHealth teams for the deployment of digital solutions
- 5. Capacity and demand management in the whole system
Key Questions to ask are:
- 1. What capabilities and competencies we have to deliver the PHM? What new skills and SME do we need to recruit?
- 2. How can the HUB teams and the BU SMEs be used to jump start the PHM strategy and then move into scaling it cluster wide?
- 3. What central resources can be pulled by clusters for the initial phase of the PHM delivery?
- 4. Who from clusters will be nominated to be part of the HHC PHM Unit?
